NOTE:

* To register your teen for Confirmation, your teen must have been baptized and received First Eucharist.

**  To register your child for Sacrament Preparation for 1st Holy Communion, they much have completed one year of 

       Faith Formation catechsis at a Parish in the 2025 - 2026 year and must be currently in grade 2 or higher. 

       Also, you must register your child for 1st - 4th Grade Class in addition to Sacrament Preparation.

MEDIA RELEASE CONSENT FORM

Occasionally pictures are taken at class, ministry events and gatherings. 

We would like to be able to use these photographs for newsletters, flyers, eBulletin, parish website, etc. 

Concerns about published pictures should be expressed to Faith Formation Director (Vicki Schwoob) 

so that it will be promptly dealt with.

AUTHORIZATION:

I authorize and give full consent, without limitations or reservations, to St. Thomas of Canterbury Parish 

to publish any photographs to which my child(ren) participating in any Faith Formation activities 

or events held at St. Thomas of Canterbury. No compensation is given.

REGISTRATION FEES:

• Registration : $125 (one child), $175 (two children), $250 (three or more children)
• First Holy Communion Fee* : $100 per child 
• Confirmation Fee** : $150 per child

*  The First Holy Communion Sacrament Preparation Fee is in addition to the Registration fee.

** Confirmation is a two-year process, hence the Confirmation Fee is in addition to the Registration fee for first year. 

In the second year, only Registration fee will be collected.

FIGURING OUT YOUR AMOUNT:

• Example of computation for one child:     2nd Grader Registration Fee + Sacrament Preparation Fee = $225
• Example of computation for Confirmation for one child year One:     Registration Fee + Confirmation Fee = $275
• All other Faith Formation Class:  Registration Fee = $125
